Business and Professions Code § 2556.2 Reporting Requirements
When an entity files a report with the California State Board of Optometry (Board) pursuant to Business and Professions Code § 2556.2, the Board requests that the report be in the following format:
- It must be on the letterhead of the optical company or registered dispensing optician (RDO), as applicable;
- It must contain the RDO license number, if applicable;
- It must be signed by a person with adequate knowledge of the entity’s operations regarding the employment of optometrists and sufficient authority to submit such a report on behalf of the entity; and
- It must contain all statutorily required information.
